API-first Healthcare

API-first healthcare is an architectural approach in which a platform's capabilities are exposed as programmable interfaces from the outset, so that connecting a new source or partner extends the system rather than forking it.

Why the ordering matters

An API added afterwards reflects the shape of the product that already exists. An API designed first reflects the shape of the domain. The difference determines whether the tenth integration is as cheap as the first, or ten times more expensive.

In healthcare this compounds unusually fast, because the number of things worth connecting — devices, records, wearables, partners — is large and grows.

One layer, not many

The failure mode is familiar: each integration acquires its own authentication, its own data handling, its own reporting path. What began as one platform quietly becomes several, and the cost of the next connection rises.

The alternative is a single layer for authentication, data, analytics and events, through which every source passes and into which every module is plugged. Sources differ; the interface does not.
